Online Survey on Attitudes, Knowledge and Beliefs about Tobacco

Published 6/12/2008 10:38:56 AM

Colorado Social Research Associates (CSRA) is helping Signal to conduct an evaluation of Signal’s Tobacco Use Recovery Now (TURN) project. As part of this evaluation, CSRA is doing an online survey of provider staff attitudes, knowledge and beliefs about tobacco use in substance abuse treatment settings. The information from this survey will be used to help evaluate the progress of the TURN project.

All staff members of Signal providers are eligible to take this anonymous survey. If you are interested, please fill out the survey by clicking on the link below. It will take between 10 and 15 minutes to complete this survey. Your participation and assistance in this effort are greatly appreciated.

You may have already taken a similar survey in either February or March of 2005. Even if you already completed a similar survey, your answers to this survey will allow us to learn more about how staff attitudes and knowledge towards tobacco use have changed over time.

To learn more about the Tobacco Use Recovery Now project, please contact Erik Stone, Director of Compliance and Quality Assurance, Signal Behavioral Health Network at (303) 639-9320.

To take the survey, click on this link. You may need to cut and paste this link into your browser. http://www.surveymonkey.com/s.aspx?sm=9r_2bUmsDJLwTj1_2bRwhJ_2bYxw_3d_3d
